What is Cytisine and how does it work?

Cytisine is the newest prescription-only smoking cessation medication to become available in the UK.

Cytisine works by mimicking the effects of nicotine. It attaches to the same receptors in the brain, effectively ‘tricking’ it into thinking that you’ve had a cigarette. This reduces the urge to smoke and helps ease withdrawal symptoms and cravings. Unlike nicotine, Cytisine is non-addictive, making it an excellent alternative to Nicotine Replacement Therapy (NRT).

Additionally, Cytisine blocks the receptors that nicotine would normally bind to, so if you smoke while using this medication, you won’t feel the usual satisfaction or reward, and you may experience side effects. For this reason, smoking is not recommended while on Cytisine.

Studies indicate that Cytisine is twice as effective as placebo in helping people to stop smoking successfully. It may be a helpful option for smokers who have previously attempted to quit or for those who found other cessation methods haven’t been effective.

How should I take Cytisine?

Cytisine is available from PrescriptionPad as 1.5mg tablets.

Before taking Cytisine, you should read the manufacturer’s printed Patient Information Leaflet (PIL) from inside the pack. It will give you more information about the tablets and provide you with a full list of the side effects which you may experience from taking them.

One package of Cytisine (100 tablets) is sufficient for a complete treatment course – the duration of therapy is 25 days.

Cytisine tablets should be swallowed whole with water and taken according to the following schedule:

Take your first dose in the morning when you wake up to help minimise cravings and withdrawal symptoms straight away. Follow the dosage schedule for the remaining tablets each day, as accurately as possible, to get the best results. Setting a timer or alarm on your phone for each dose can be a helpful reminder.

Smoking should be stopped no later than on the 5th day of treatment. Smoking should not be continued during treatment as this may aggravate adverse reactions.

To achieve the best results, make sure to complete the entire pack of tablets. If you still experience cravings or need additional support after finishing the treatment, consult your doctor.

In case of treatment failure, the treatment should be discontinued and may be resumed after 2 to 3 months.

If you forget to take a dose, leave out the missed dose but make sure that you remember to take your next dose when it is due. Do not take two doses together to make up for the forgotten dose.

If you accidentally take more Cytisine than prescribed, you may experience nicotine intoxication. Symptoms of overdose include malaise, nausea, vomiting, increased heart rate, fluctuations in blood pressure, breathing problems, blurred vision, and convulsions. If you experience any of these, you should stop taking Cytisine and contact a doctor immediately.

Along with their useful effects, most medicines can cause unwanted side effects although not everyone experiences them. The unwanted effects often improve as your body adjusts to the new medicine, but please speak with your doctor or pharmacist if they continue or become troublesome.

  • Change in appetite (usually an increase)
  • Weight gain
  • Dizziness
  • Irritability
  • Mood changes
  • Anxiety
  • Increased blood pressure (hypertension)
  • Dry mouth
  • Diarrhoea
  • Rash
  • Fatigue
  • Sleep disorders (insomnia, drowsiness, lethargy, abnormal dreams, nightmares)
  • Headaches
  • Increased heart rate
  • Nausea
  • Change in taste
  • Heartburn
  • Constipation
  • Vomiting
  • Abdominal pain (especially in the upper abdomen)
  • Muscle pain

 

Common side effects (may affect up to 1 in 10 people)

  • Difficulty in concentration
  • Slow heart rate
  • Abdominal distension
  • Burning tongue
  • Malaise

 

Effects of giving up smoking

People who stop smoking are often affected by nicotine withdrawal. This can also affect people taking Cytisine. Signs of nicotine withdrawal include: difficulty sleeping, tremor or sweating, feeling anxious, agitated or depressed, sometimes with thoughts of suicide. You should talk to your doctor if you have any concerns about how you feel.

Please tell the prescriber if you are taking, have recently taken or might take any other medicines, including medicines obtained without a prescription. This is because Cytisine can affect the way some medicines work, and some medicines can have an effect on Cytisine. In the event of a medical emergency, you should tell your doctor, pharmacist, or nurse that you have taken Cytisine tablets and when you did. Do not take Cytisine tablets with other medicines unless your doctor tells you that you can.

  • Do not take Cytisine with anti-tuberculosis drugs

 

  • It is currently unknown whether Cytisine may reduce the effectiveness of systemically acting hormonal contraceptives. If you are using systemically acting hormonal contraceptives, you should add a second barrier method (e.g. condoms)

 

  • The dose of certain medications may need to be adjusted when you stop smoking. The chemicals absorbed through smoking can reduce the effectiveness of some medicines, so when you quit, your dosage might need to be lowered to avoid potential side effects. If you’re taking any other medications, consult your doctor if you experience any new symptoms that could be side effects

Do not take Cytisine:

  • If you are allergic (hypersensitive) to Cytisine or any of the other ingredients listed in the Patient Information Leaflet supplied with your medication
  • If you have unstable angina
  • If you have a recent history of heart attack
  • If you have heart arrhythmias (irregular or abnormal pulse)
  • If you have had a recent stroke or TIA
  • If you are pregnant or breastfeeding

 

Warnings & Precautions

Cytisine should be used with caution in case of ischemic heart disease, heart failure, hypertension, pheochromocytoma (a tumour of the adrenal gland), atherosclerosis (hardening of the arteries) and other peripheral vascular diseases, gastric and duodenal ulcer, gastroesophageal reflux disease, hyperthyroidism (overactive thyroid), diabetes, schizophrenia, kidney and liver failure. Cytisine should be taken only by those with a serious intention of weaning off nicotine. The use of Cytisine and continuation of smoking could lead to aggravated side effects of nicotine.

Due to limited clinical experience, Cytisine is not recommended for use in elderly patients over 65 years of age, or patients with kidney and liver impairment.

Stopping smoking – the effects of changes in your body resulting from stopping smoking, with or without treatment with Cytisine, may alter the way other medicines act. Therefore, in some cases an adjustment of the dose may be necessary. For some people, stopping smoking with or without treatment has been associated with an increased risk of experiencing changes in thinking or behaviour, feelings of depression and anxiety (rarely including suicidal ideation and suicide attempt) and can be associated with a worsening of psychiatric disorder. If you have a history of psychiatric disorder you should discuss this with your doctor.

 

Pregnancy, Breastfeeding, & Fertility

Cytisine should be avoided during pregnancy and when breastfeeding.

Women of childbearing potential must use highly effective contraception while taking Cytisine. Women using systemically acting hormonal contraceptives should add a second barrier method.

 

Driving and using machinery

Cytisine has no known influence on the ability to drive and use machines.
