You should not use Mounjaro if you are allergic to any of the ingredients in the injection, especially Tirzepatide. Additionally, Mounjaro is not suitable for individuals under the age of 18, and it is not recommended for those who are pregnant or breastfeeding. You should stop taking Mounjaro at least one month before trying to get pregnant. These restrictions are in place because weight loss medications can lead to serious health risks and severe side effects when used inappropriately or in unsuitable populations.

Mounjaro starts to work in your body immediately, though it might take a few weeks until you notice the full effect of its impact on your appetite and eating habits. The dosage will be increased gradually to minimise side effects. Data from clinical trials suggests that initial weight loss of around 2kg can be observed as early as 4 weeks after starting treatment.

Mounjaro is designed as a long-term treatment – you should expect to see significant weight loss results from around 6 months with continuous use, with maximum weight loss generally occurring between 12 and 18 months (depending on dosage levels and other lifestyle adjustments).

You’ll get the best effect from this medication when taking it alongside healthy lifestyle changes, such as eating a balanced diet, reducing snacking between meals, and exercising for 150 minutes a week.

It’s important to remember that everyone responds differently to treatment, and there’s no rush to increase your dose. You should only increase your dosage when you feel ready. If you’d prefer to stay at the same dose for an additional month or two, or even permanently, that’s perfectly fine! You can also decrease the dose if you find that you’re not tolerating a higher dose well after your first month.

Dose increases should only happen if you’re tolerating your current dose without side effects and want to accelerate your weight loss. However, if you’re happy with your current weight loss and tolerating the dose well, it’s completely acceptable to remain on that dose for as long as necessary. Typically, the “maintenance doses” are 5mg, 10mg, or 15mg. If you experience any side effects, you should either stay on your current dose or consider reducing it. You inject Mounjaro under the skin (subcutaneously) of your stomach, thigh, or upper arm. Don’t worry, it’s actually much simpler than it sounds and if we prescribe Mounjaro for you, we’ll provide you with clear, step-by-step instructions on how to use it.

You should inject your dose of Mounjaro once a week, on the same day and at the same time. This helps keep the levels of Tirzepatide steady in your bloodstream, reducing the risk of side effects and enhancing its effectiveness.

It’s important to change injection sites regularly to prevent skin irritation. You can inject in the same general area, but be sure to choose a different spot in that area each time. Always use a new needle for each injection and make sure to use a sharps bin for safe disposal.

You should check your pen and needle for signs of damage and do not use it if it’s unsealed or bent. You should also check the liquid for discolouration or cloudiness; if it isn’t clear or slightly yellowish, you may need to use a new pen.

Before administering your dose, make sure that you’ve got the correct medication and dose according to the Mounjaro dosing schedule. This is especially important if you take other injectable medications like insulin.

 

Mounjaro KwikPen Usage Instructions:

  1. Wash your hands with soap and water.
  2. Inspect your pen and label. Check the expiry date and that the pen contains the right dose. Check the liquid inside is colourless to a slightly yellow tone. Don’t use the pen if the liquid has been frozen, has particles in it, is discoloured or cloudy. Don’t use the medication if the name or dose doesn’t match your prescription, or if the pen is expired or damaged.
  3. Pull off the grey base cap. This should come off easily.
  4. Wipe the red inner seal with a swab. Please note that alcohol swabs are not provided.
  5. Select a new pen needle. Always use a new needle for each injection to help prevent infections and blocked needles. Pull off the paper tab from the outer needle shield.
  6. Push the capped pen needle straight onto the pen and twist the pen needle until it is tight. 
  7. Pull off the outer needle cap and keep it, this will be reused. Pull off the inner needle cap, this can go in the bin.
  8. Slowly turn the knob dose until you hear 2 clicks and the extended line is shown in the dose window. This is the prime position.
  9. Hold your pen with the needle pointing upwards and tap the cartridge holder gently to collect air bubbles at the top. Then release some medicine into the air by pushing the dose knob in until it stops. Slowly count to 5 while you do this. The 0 icon must be shown in the dose window.
  10. Choose an injection site. Mounjaro can be injected in your lower stomach, thighs, or back of your upper arms. You can use the same area of your body every week but be sure to change your injection site. 
  11. Turn the dose knob until it stops and the 1 icon is shown in the dose window. This is equal to a full 0.6ml dose.
  12. Insert the needle into your skin. Inject the medicine by pushing the dose knob in until it stops then slowly count to 5 while holding the dose knob. The 0 icon must be shown in the dose window before removing the needle.
  13. Pull the needle out of your skin, it’s normal to see a drop of medicine on the needle. If you see some blood lightly press the area with cotton wool.
  14. Check that the 0 icon is in the dose window, if it is then you’ve received the full dose. If you don’t see the icon in the dose window, reinsert the needle and finish the injection.
  15. Replace the outer needle shield.
  16. Unscrew the capped needle and put it in your sharps bin. Then replace the pen cap.  

Along with their useful effects, most medicines can cause unwanted side effects although not everyone experiences them. The unwanted effects often improve as your body adjusts to the new medicine, but please speak with your doctor or pharmacist if they continue or become troublesome. You will find a full list of possible side effects in the Patient Information Leaflet (PIL) supplied with your medicine.

The side effects of Mounjaro are typically mild and tend to improve over time. They are most likely to occur when you first start taking Mounjaro or during dose increases. To help minimise these effects, Mounjaro is introduced gradually, giving your body time to adjust to the treatment. This approach helps ensure a smoother experience while using the medication.

 

Stop using Mounjaro and seek medical help immediately if you experience any of the following serious side effects:

  • Uncommon (may affect up to 1 in 100 people) – Inflamed pancreas (acute pancreatitis) which could cause severe pain in the stomach and/or back which does not go away, with or without persistent vomiting. You should see a doctor immediately if you experience such symptoms.
  • Rare (may affect up to 1 in 1000 people) – Severe allergic reactions (e.g. anaphylactic reaction, angioedema). You should get immediate medical help and inform your doctor if you experience symptoms such as breathing problems, rapid swelling of the face, lips, tongue and/or throat with difficulty swallowing and a fast heartbeat.

 

Very common side effects (may affect more than 1 in 10 people)

  • Nausea
  • Diarrhoea
  • Vomiting
  • Constipation
  • Low blood sugar (hypoglycaemia) – if taken alongside certain diabetes medications

 

Mounjaro can cause low blood sugar when used with other medicines for type 2 diabetes, such as insulin and sulphonylureas. Tell your doctor if you get symptoms of low blood sugar, which include headache, drowsiness, weakness, dizziness, feeling hungry, confusion, shaking, sweating, irritability, and an increased heart rate.

 

Common side effects (may affect up to 1 in 10 people)

  • Allergic reaction/hypersensitivity (e.g. rash, itching, and eczema)
  • Dizziness, feeling lightheaded, headache
  • Low blood pressure
  • Decreased appetite
  • Stomach (abdominal) pain
  • Acid reflux and indigestion symptoms, bloating, gas, burping
  • Hair loss
  • Fatigue/tiredness
  • Injection site reactions (e.g. irritation, itching, or redness)
  • Fast pulse
  • Increased levels of pancreatic enzymes (such as lipase and amylase) in blood

 

Uncommon side effects (may affect up to 1 in 100 people)

  • Gallstones
  • Injection site pain
  • Increased calcitonin levels in blood
  • Cholecystitis (infection of the gallbladder)
  • Changed sense of taste

Common Mounjaro side effects are usually mild and are likely to lessen or even disappear after your body gets used to the treatment. We understand that it’s still tough to live with these discomforts though and so we’ve put together some tips for managing these where possible:

  • Make sure to inject yourself at the same time and day each week so your body has a regular interval between each dose
  • Only take the dose prescribed to you and avoid increasing your dose unless you are told to do so by a healthcare professional
  • Stay well hydrated
  • Avoid fatty foods and heavy meals
  • Stop eating when you feel full
  • Exercise regularly
  • Manage nausea, vomiting, and diarrhoea symptoms by sipping water, avoiding greasy, heavy meals, and eating smaller meals more regularly. It may also help to avoid anything that smells or tastes too strong. You can take anti-diarrhoea tablets if needed
  • If you’re struggling with constipation, increase your fibre intake, stay well hydrated and try eating small fruits such as prunes, figs, or grapes. Make sure you’re also getting regular activity, such as going on short walks
  • To ease a headache, spend time in a quiet space to relax, take a simple pain relief medicine like paracetamol, drink plenty of water, and try relaxation techniques such as meditation and scalp massage

 

It is important to inform your doctor if you experience any side effects that persist or cause discomfort. They can provide guidance on how to manage these symptoms or adjust your treatment plan if necessary.

You should store your Mounjaro pen in the refrigerator at a temperature of 2°C to 8°C before use. Once opened, you can either continue storing it in the refrigerator or in a cool, dark place away from direct sunlight, as long as the temperature remains below 30°C.

Used pens should be discarded no later than 30 days after first use. Always keep the pen cap on when the pen is not in use to protect it from light exposure.

Important reminders:

  • Do not store Mounjaro in the freezer. If the medication inside freezes, it should not be used, even after thawing
  • Dispose of needles in a sharps bin after use
  • Always keep medicines out of reach of children and vulnerable adults

The starting dose of Mounjaro is always 2.5mg for the first 4 weeks. Even if you’re switching from another weight loss injection or have taken Mounjaro in the past, you’ll still begin with this dose.

The dosage will then increase every four weeks (or as advised by your prescriber) until you reach your maximum maintenance dose of 5mg, 10mg, or 15mg. The highest dose of 15mg generally leads to the most significant weight loss, but results depend on how your body responds to the medication and any side effects that you may experience.

Important: Never increase your dose beyond what your prescriber recommends, as this increases the risk of severe side effects, potential illness, and could cause you to pause or stop treatment, which may slow your weight loss progress.

  • If it’s been 4 days or less since your missed dose, take it as soon as you remember. You can then continue with your regular dosing schedule, even if this means taking two doses just a few days apart
  • If it’s been more than 4 days, skip the missed dose and take your next dose on your usual scheduled day. Be sure not to take two doses within 3 days of each other

When you stop taking Mounjaro, the effects of the medication will gradually wear off. This means you may start feeling hungry again, and if you were using Mounjaro to manage blood sugar levels, they are likely to return to their previous state. It’s important to consult your prescriber before stopping Mounjaro, as they may recommend gradually reducing your dose to avoid abrupt changes.

Stopping Mounjaro too soon could lead to regaining any weight you’ve lost. To maximise the benefits, it’s best to follow the dosing schedule and consult your prescriber if you experience side effects, as they may adjust your dose to help you stay on track.

Everyone responds to Mounjaro differently, and weight loss can take time. Studies show that weight loss with Mounjaro is often achieved over a longer period. In a 72-week study, participants lost up to 20% of their body weight over 16.5 months.

No, Mounjaro and Wegovy are not the same, although they both work similarly as weekly injectable weight loss treatments. Mounjaro contains the active ingredient Tirzepatide, while Wegovy contains Semaglutide. Zepbound and Mounjaro are essentially the same medication, both containing the active ingredient Tirzepatide and working in the same way for weight loss management. When starting treatment with Mounjaro, in some cases you may experience loss of fluids/dehydration, e.g. due to vomiting, nausea and/or diarrhoea, which may lead to a decrease in kidney function. It is important to avoid dehydration by drinking plenty of fluids. Contact your doctor if you have any questions or concerns.

 

Pregnancy, Breastfeeding, & Fertility

  • This medicine should not be used during pregnancy as the effects of this medicine on an unborn child are not known. It is recommended to use contraception while using this medicine. If you are a woman with obesity or overweight and are using oral contraceptives, you should consider also using a barrier method of contraception or switching to a non-oral contraceptive method for 4 weeks after starting Mounjaro and for 4 weeks after each increase in dose
  • Mounjaro is not recommended for use while breastfeeding

 

Driving and using machinery

It is unlikely that this medicine will affect your ability to drive and use machinery. However, if you use Mounjaro in combination with a Sulphonylurea or Insulin, low blood sugar (hypoglycaemia) may occur which may reduce your ability to concentrate. Avoid driving or using machines if you get any signs of low blood sugar, e.g. headache, drowsiness, weakness, dizziness, feeling hungry, confusion, irritability, fast heartbeat and sweating.
