What is Nitrofurantoin and how does it work?

Nitrofurantoin is a broad-spectrum antibiotic that is used to prevent and treat infections of the bladder, kidney and other parts of the urinary system.

How should I take Nitrofurantoin?

Before taking Nitrofurantoin, you should read the manufacturer’s printed Patient Information Leaflet (PIL) from inside the pack. It will give you more information about the capsules and provide you with a full list of the side effects which you may experience from taking them.

The recommended dose for lower urinary-tract infections, using modified-release capsules is 100mg twice daily. Capsules should be swallowed whole with food or milk to avoid stomach upset and to aid absorption.

Most people are prescribed a short course of treatment, which usually lasts between 3-7 days. It is important to complete the course even if you feel better to prevent the infection coming back.

If you forget to take a dose at the right time, take it as soon as you remember. Try to take the correct number of doses each day but do not take two doses at the same time to make up for a missed dose.

Along with their useful effects, most medicines can cause unwanted side effects although not everyone experiences them. The unwanted effects often improve as your body adjusts to the new medicine, but please speak with your doctor or pharmacist if they continue or become troublesome. You will find a full list of possible side effects in the Patient Information Leaflet (PIL) supplied with your medicine.

Stop taking Nitrofurantoin and seek immediate medical attention if:

  • You notice any sudden wheeziness, difficulty in breathing, swelling of the eyelids, face or lips, rash or itching (especially affecting your whole body)
  • Your lungs react to Nitrofurantoin. This may develop quickly, within a week of starting treatment or very slowly, especially in elderly patients. This may produce fever, chills, cough, and shortness of breath
  • Nitrofurantoin causes the liver to become inflamed, producing jaundice (yellowing of the skin or whites of the eyes)
  • You develop a severe reduction in blood cells which can cause weakness, bruising or make infections more likely
  • You notice any blue or purple colouration of the skin due to low oxygen levels. A condition known as cyanosis
  • You develop symptoms of fever, flu, abdominal pain, diarrhoea, blood in your stool and weakness. These could be signs of a condition known as cutaneous vasculitis
  • You develop symptoms of fatigue, abdominal pain, joint pain and swelling. These could be signs of a condition known as hepatitis

Please tell the prescriber if you are taking, have recently taken or might take any other medicines, including medicines obtained without a prescription. This is because Nitrofurantoin can affect the way some medicines work, and some medicines can have an effect on Nitrofurantoin. In the event of a medical emergency, you should tell your doctor, pharmacist or nurse that you have taken Nitrofurantoin and when you did. Do not take Nitrofurantoin with other medicines unless your doctor tells you that you can.

Tell your doctor or pharmacist if you are taking any of the following medicines:

  • Indigestion treatments (e.g. Magnesium Trisilicate)
  • Medicines for gout (e.g. Probenecid or Sulfinpyrazone)
  • Medicines for raised pressure in the eye (glaucoma) such as Carbonic Anhydrase Inhibitors
  • Medicines which make the urine less acidic (e.g. Potassium Citrate)
  • Antibiotics known as Quinolones
  • Currently undergoing treatment with the oral Typhoid Vaccine

Do not take Nitrofurantoin:

  • If you are allergic (hypersensitive) to Nitrofurantoin or any of the ingredients listed in the Patient Information Leaflet supplied with your medication
  • If you have a disease of the kidneys which is severely affecting the way they work (ask your doctor if you are not sure)
  • If you are in the final stages of pregnancy (labour or delivery) as there is a risk that it might affect the baby
  • If you have porphyria (a rare inherited disorder)
  • If you have Glucose-6-Phosphate Dehydrogenase (G6PD) deficiency or are breastfeeding a baby with suspected or known G6PD deficiency

 

Warnings & Precautions

Nitrofurantoin can cause your urine to turn a yellow/brown colour. This is harmless and not a reason to stop taking it.

Talk to your doctor, pharmacist, or nurse before taking Nitrofurantoin:

  • If you have diabetes
  • If you are suffering from any illness causing severe weakness
  • If you have anaemia (a decrease in red blood cells causing pale skin, weakness and breathlessness), a lack of Vitamin B, or abnormal levels of salts in your blood (your doctor will be able to advise you)
  • If you have a history of allergic reactions
  • If you have any problems with your kidneys
  • If you have any disease of the lungs, liver or nervous system
  • If you have false positive results on testing your urine for glucose
  • If you have urine coloured yellow or brown
  • If you experience fatigue, yellowing of the skin or eyes, itching, skin rashes, joint pain, abdominal discomfort, nausea, vomiting, loss of appetite, dark urine, and pale or grey-coloured stools. These may be symptoms of a liver disorder

 

Pregnancy, Breastfeeding, & Fertility

  • Nitrofurantoin may be taken during pregnancy; however, it should not be used during labour or delivery because there is a possibility that use at this stage may affect the baby
  • Nitrofurantoin should be avoided in breastfeeding due to the risk of haemolysis in G6PD-deficient infants

 

Driving and using machinery

Nitrofurantoin may cause dizziness and drowsiness. You should not drive or operate machinery if you are affected.
