What is Orlistat and how does it work?

Orlistat is a weight loss treatment that is used to help you lose weight if you are overweight or obese.

Orlistat works by interfering with the way fat is digested and absorbed by your body. It attaches to and prevents the action of lipase enzymes found in the digestive juices of the stomach and small intestine. These enzymes normally break down the fat which you consume in your diet, allowing it to be absorbed into your bloodstream. By inhibiting these enzymes, Orlistat reduces the digestion of fat, allowing it to be passed through the body instead of being absorbed. This reduction in dietary fat absorption aids in weight loss.

Orlistat only prevents about a third of the fat you eat from being absorbed and only works effectively alongside a reduced calorie diet and increased physical activity levels.

Orlistat is licensed for use in adults with a Body Mass Index (BMI) of 30 or higher, or in individuals with a BMI of 28 or more who also have at least one weight-related risk factor (e.g. controlled high blood pressure, diabetes, high cholesterol…).

How should I take Orlistat?

Before taking Orlistat, you should read the manufacturer’s printed Patient Information Leaflet (PIL) from inside the pack. It will give you more information about Orlistat and provide you with a full list of the side effects which you may experience from taking it.

The recommended dose of Orlistat is one 120mg capsule taken with each of your three main meals of the day. No more than three 120mg capsules should be taken in 24 hours. Capsules should be swallowed with water immediately before, during, or up to one hour after each main meal. If a meal is missed or contains no fat, the dose of Orlistat should be omitted.

Patients should be on a nutritionally balanced, calorie-controlled diet that contains approximately 30% of calories from fat. It is recommended that the diet should be rich in fruit and vegetables. The daily intake of fat, carbohydrates, and protein should be distributed over three main meals. To gain optimal benefit, avoid the intake of food containing fat between meals, such as biscuits, chocolate, and savoury snacks.

Orlistat may reduce the absorption of fat-soluble vitamins (A, D, E, and K) in your body. To counter this, it’s recommended to take a daily multivitamin containing these vitamins while using Orlistat. For best results, take the multivitamin at bedtime, separate from your Orlistat doses, to help ensure proper absorption of the vitamins.

Treatment with Orlistat should be discontinued after 12 weeks if patients have been unable to lose at least 5% of their body weight as measured at the start of treatment.

Along with their useful effects, most medicines can cause unwanted side effects although not everyone experiences them. The unwanted effects often improve as your body adjusts to the new medicine, but please speak with your doctor or pharmacist if they continue or become troublesome. You will find a full list of possible side effects in the Patient Information Leaflet (PIL) supplied with your medicine.

The majority of unwanted effects related to the use of Orlistat result from its local action in your digestive system. These symptoms are generally mild, occur at the beginning of treatment, and are particularly experienced after meals containing high levels of fat. Normally, these symptoms disappear if you continue treatment and keep to your recommended diet.

Very common side effects (may affect more than 1 in 10 people)

  • Headache
  • Abdominal pain/discomfort
  • Urgent or increased need to open the bowels
  • Flatulence (wind) with discharge
  • Oily discharge
  • Oily or fatty stools
  • Liquid stools
  • Low blood sugar levels (experienced by some people with type 2 diabetes)

 

Common side effects (may affect up to 1 in 10 people)

  • Rectal pain/discomfort
  • Soft stools
  • Incontinence
  • Bloating
  • Tooth/gum disorders
  • Irregularity of menstrual cycle
  • Tiredness

Please tell the prescriber if you are taking, have recently taken, or might take any other medicines, including medicines obtained without a prescription. This is because Orlistat can affect the way some medicines work, and some medicines can have an effect on Orlistat. In the event of a medical emergency, you should tell your doctor, pharmacist, or nurse that you have taken Orlistat and when you did. Do not take Orlistat with other medicines unless your doctor tells you that you can.

Do not take Orlistat with:

  • Anticoagulant medicines (e.g. Warfarin)
  • Ciclosporin

 

Tell your doctor or pharmacist if you are taking any of the following medicines:

  • Acarbose
  • Levothyroxine and/or Iodine salts
  • Amiodarone
  • Anticonvulsants or antiepileptic medicines – Orlistat may reduce the absorption of these medicines, please report any changes in the frequency and severity of your convulsions with your doctor
  • Medicines to treat HIV
  • Medicines for depression, psychiatric disorders, or anxiousness

 

Orlistat can interfere with the absorption of the contraceptive pill. If you take ‘the pill’ and you have severe diarrhoea as a side-effect of taking Orlistat, it could reduce the effectiveness of the contraception. If you experience diarrhoea, use another method of contraception in addition to the pill – for example, a condom.

Orlistat can also interfere with the absorption of vitamins which are fat-soluble. These are vitamins A, D, E, and K. If you decide to take a multivitamin supplement, take it at a time when you are not also taking Orlistat. The best time to take the supplement would be at bedtime.

Do not take Orlistat:

  • If you are allergic or hypersensitive to Orlistat, or any of the ingredients listed in the Patient Information Leaflet supplied with your medication
  • If you have chronic malabsorption syndrome (insufficient absorption of nutrients from the gastrointestinal tract)
  • If you have cholestasis (liver disorder)
  • If you are breastfeeding

 

Warnings & Precautions

  • Weight loss can impact the dosage of medications you take for other conditions, such as high cholesterol or diabetes. It’s important to discuss any medications you’re currently using with your doctor, as losing weight may require adjustments to your prescribed doses to ensure continued effectiveness and safety.

 

  • To gain the maximum benefit from Orlistat, you should follow a nutritionally balanced, calorie-controlled diet that contains approximately 30% of calories from fat. It is recommended that the diet should be rich in fruit and vegetables.

 

  • This medicine may cause harmless changes in your bowel habits, such as fatty or oily stools, as a result of undigested fat being eliminated in your stool. This is more likely to occur if Orlistat is taken with a diet high in fat. To reduce the risk of this side effect, it’s recommended that your daily fat intake be evenly distributed across three main meals. Taking Orlistat with a particularly high-fat meal may increase the likelihood of gastrointestinal effects.

 

  • It is recommended to use an additional method of contraception to prevent potential failure of oral contraceptives, which may occur in the event of severe diarrhoea.

 

  • The use of Orlistat may be linked to the development of kidney stones in patients with chronic kidney disease. Be sure to inform your doctor if you have any kidney-related issues before starting treatment.

 

Pregnancy, Breastfeeding, & Fertility

Orlistat should not be used during pregnancy or while breastfeeding.

 

Driving and using machinery

Orlistat has no known effect on your ability to drive a vehicle or operate machinery.
