What is Pantoprazole and how does it work?

Pantoprazole belongs to a group of medicines called Proton Pump Inhibitors (PPIs) which reduce the amount of acid produced in the stomach by blocking the ‘proton pump’ of parietal cells in the lining of the stomach. This targeted mechanism of action on the final step of acid formation provides for highly effective results.

PPIs are widely used for the treatment of acid reflux, heartburn and indigestion. They can also be used to help prevent ulcers from forming or to assist healing in cases where ulcer formation has already occurred. PPIs can be given in combination with appropriate antibiotics for the treatment of Helicobacter pylori (a type of bacteria that can cause ulcers in the lining of the stomach by weakening the protective mucus coating of the stomach). PPIs are sometimes taken for a rare illness called Zollinger-Ellison syndrome in which the stomach produces too much acid due to a tumour in the pancreas or upper part of the small intestine.

How should I take Pantoprazole?

The usual adult dose is 20-40mg once daily. It is recommended that you take Pantoprazole one hour before a meal. Pantoprazole tablets should be swallowed whole with some water, do not chew or break the tablets.

If you accidentally take more Pantoprazole than prescribed, it is very unlikely to cause any problems. However, you should talk to your doctor or pharmacist straight away if you have taken too many and have any of the following symptoms: flushed skin, feeling sweaty, a fast heartbeat, feeling sleepy, blurred vision, feeling confused or agitated.

If you forget to take a dose at your usual time, you can take it as soon as you remember it. However, if it is almost time for your next dose, skip the missed dose. Do not take a double dose to make up for a forgotten dose.

Along with their useful effects, most medicines can cause unwanted side effects although not everyone experiences them. The unwanted effects often improve as your body adjusts to the new medicine, but please speak with your doctor or pharmacist if they continue or become troublesome.

Below is a list of the most common and the most serious side effects associated with Pantoprazole. You will find a full list of possible side effects in the Patient Information Leaflet (PIL) supplied with your medicine.

Common side effects

Most people who take Pantoprazole do not suffer any side effects. The following more common side effects tend to be mild and abate when you stop taking Pantoprazole.

  • Headache
  • Effects on your stomach or gut: diarrhoea, stomach pain, constipation, wind/flatulence
  • Nausea and vomiting
  • Benign polyps in the stomach

 

If you notice any of the following rare but serious side effects, stop taking Pantoprazole and contact a doctor immediately:

  • Serious allergic reactions: swelling of the tongue and/or throat, difficulty in swallowing, hives (nettle rash), difficulties in breathing, allergic facial swelling (Quincke’s oedema / angioedema), severe dizziness with very fast heartbeat and heavy sweating
  • Serious skin conditions: blistering of the skin and rapid deterioration of your general condition, erosion (including slight bleeding) of eyes, nose, mouth/lips or genitals (Stevens-Johnson-Syndrome, Lyell-Syndrome, Erythema multiforme) and sensitivity to light
  • Other serious conditions: yellowing of the skin or whites of the eyes, dark urine and tiredness (severe damage to liver cells/jaundice), fever/rash sometimes with painful urination, reduced urination, swollen ankles and lower back pain (serious inflammation of the kidneys possibly leading to kidney failure)

Please tell the prescriber if you are taking, have recently taken or might take any other medicines, including medicines obtained without a prescription. This is because Pantoprazole can affect the way some medicines work, and some medicines can have an effect on Pantoprazole.

Tell us if you are taking any of the following medicines:

  • Ketoconazole, Itraconazole and Posaconazole (used to treat fungal infections) or Erlotinib (used for certain types of cancer) because Pantoprazole may stop these and other medicines from working properly
  • Warfarin and Phenprocoumon, which affect the thickening, or thinning of the blood. You may need further checks
  • Medicines used to treat HIV-infection, such as Atazanavir
  • Methotrexate (used to treat rheumatoid arthritis, psoriasis, and cancer) – if you are taking Methotrexate your doctor may temporarily stop your Pantoprazole treatment because Pantoprazole can increase levels of Methotrexate in the blood
  • Fluvoxamine (used to treat depression and other psychiatric diseases) – if you are taking Fluvoxamine your doctor may reduce the dose
  • Rifampicin (used to treat infections)
  • St John’s wort [Hypericum perforatum] (used to treat mild depression)

Do not take Pantoprazole:

  • If you are allergic (hypersensitive) to Pantoprazole or any of the other ingredients of Pantoprazole (listed in the Patient Information Leaflet supplied with your medication)
  • If you are allergic to medicines containing other Proton Pump Inhibitors (e.g. Omeprazole, Lansoprazole, Rabeprazole, Esomeprazole)
  • If you are due to have an endoscopy in the next two weeks

 

Pantoprazole is not usually recommended during pregnancy and while breastfeeding. If you are pregnant or breastfeeding, it is always better to try to treat your symptoms without taking medication when possible. A medicine called Omeprazole, which is similar to Pantoprazole, is considered safe in pregnancy/breastfeeding so may be a more suitable alternative.

 

Warnings & Precautions

Pantoprazole may hide the symptoms of other diseases. Therefore, if any of the following happen to you before you start taking Pantoprazole or whilst you are taking it, talk to your doctor straightaway:

  • You lose a lot of weight for no reason and have problems swallowing
  • You begin to vomit food or vomit blood
  • You pass black stools (blood-stained faeces)
  • You experience severe or persistent diarrhoea, with symptoms such as fever, abdominal pain or tenderness, as Pantoprazole has been associated with a small increase in infectious diarrhoea
  • You have severe liver problems
  • You experience stomach pain that seems to be getting worse – this can be a sign of an inflamed liver or pancreas
  • You have ever had a skin reaction after treatment with a medicine similar to Pantoprazole that reduces stomach acid

 

Taking a proton pump inhibitor like Pantoprazole, especially over a period of more than one year, may slightly increase your risk of fracture in the hip, wrist or spine. Tell the prescriber if you have osteoporosis or if you are taking corticosteroids (which can increase the risk of osteoporosis).

If you get a rash on your skin, especially in areas exposed to the sun tell your doctor as soon as you can as you may need to stop your treatment with Pantoprazole. Remember to also mention any other ill-effects like pain in your joints.

If you take Pantoprazole for more than three months, it is possible that the levels of magnesium in your blood may fall. Low levels of magnesium can cause fatigue, involuntary muscle contractions, disorientation, convulsions, dizziness, and increased heart rate. If you get any of these symptoms, please tell your doctor promptly. Low levels of magnesium can also lead to a reduction in potassium or calcium levels in the blood. Your doctor may decide to perform regular blood tests to monitor your levels of magnesium.

As with all acid reducing agents, Pantoprazole may lead to a reduced absorption of Vitamin B12.

Tell your doctor before taking this medicine if you are due to have a specific blood test (Chromogranin A).

 

Driving and using machinery

Pantoprazole has no or negligible influence on the ability to drive and use machines. If you experience side effects like dizziness or blurred vision, you should not drive or operate machinery.
