What is Sildenafil and how does it work?

Sildenafil belongs to a group of medicines called Phosphodiesterase Type 5 (PDE5) inhibitors which are used as a first-line treatment option for Erectile Dysfunction (ED).

PDE5 inhibitors function by relaxing and widening (dilating) blood vessels in your penis. This helps to improve blood flow to the penis when you are sexually aroused. Even if the nerves or blood vessels going to the penis are not working so well, PDE5 inhibitors can increase the blood flow and facilitate an erection.

PDE5 inhibitors have about an 8 in 10 chance of working well, however, it should be noted that they will not cause an erection unless you are sexually aroused or stimulated.

How should I take Sildenafil?

Sildenafil is available from PrescriptionPad as 25mg, 50mg, and 100mg Tablets.

Before taking Sildenafil, you should read the manufacturer’s printed Patient Information Leaflet (PIL) from inside the pack. It will give you more information about the tablets and provide you with a full list of the side effects which you may experience from taking them.

The usual recommended starting dose is 50mg. This can be increased to 100mg or decreased to 25mg depending on its effect and how you react.

The amount of time that Sildenafil takes to work varies from person to person, but it normally takes between half an hour to one hour.

Sildenafil is intended to be taken before sexual activity – it is NOT for continuous daily use. You should take one tablet about an hour before you plan to have sex, although it may still be effective up to six hours after taking the tablet. You should not take more than one dose of Sildenafil in any 24-hour period.

You and your partner will still need to engage in foreplay, just as you would if you were not taking a medicine for ED. Sildenafil will not cause an erection unless you are sexually aroused.

Tablets should be swallowed whole with a glass of water or juice (but not grapefruit juice). It is recommended that you do not drink grapefruit juice with Sildenafil as there is a chemical in grapefruit juice which can increase the amount of Sildenafil in your bloodstream and this makes side-effects more likely.

Sildenafil can be taken with or without food. However, you may find that it takes longer to start working if you take it with a heavy meal.

You should not drink large amounts of alcohol before you plan to take Sildenafil. Drinking too much alcohol can reduce your ability to get an erection and this may prevent you from getting the maximum benefit from the tablets.

You should not take Sildenafil if you are using any other products or taking any other medicines to treat Erectile Dysfunction.

If Sildenafil tablets do not help you to get an erection or if your erection does not last long enough for satisfactory sexual intercourse, you should inform your doctor. Similarly, if you feel that the tablets are too strong for you, discuss this with your doctor as your dose may need to be reduced.

If you accidently take more Sildenafil than prescribed, you may experience an increase in side effects and their severity. Doses above 100mg do not increase the efficacy. Contact your doctor if you take more tablets than you should.

Along with their useful effects, most medicines can cause unwanted side effects although not everyone experiences them. The unwanted effects often improve as your body adjusts to the new medicine, but please speak with your doctor or pharmacist if they continue or become troublesome.

Very common side effects (may affect more than 1 in 10 people)

  • Headache

 

Common side effects (may affect up to 1 in 10 people)

The following more common side effects tend to be mild and of a short duration.

  • Facial flushing
  • Hot flush (symptoms include a sudden feeling of heat in your upper body)
  • Nausea
  • Indigestion
  • Visual disturbances (colour tinge to vision, blurred vision)
  • Stuffy nose
  • Dizziness

 

If you notice any of the following rare but serious side effects, stop taking Sildenafil and contact a doctor immediately:

  • An allergic reaction – symptoms include sudden wheeziness, difficulty in breathing or dizziness, swelling of the eyelids, face, lips or throat
  • Chest pains – if this occurs during or after intercourse get in a semi-sitting position and try to relax. Do not use Nitrates to treat your chest pain
  • Prolonged and sometimes painful erections – if you have an erection which lasts for more than 4 hours, you should contact a doctor immediately
  • A sudden decrease or loss of vision
  • Serious skin reactions – symptoms may include severe peeling and swelling of the skin, blistering of the mouth, genitals and around the eyes
  • Seizures or fits

 

Please tell the prescriber if you are taking, have recently taken, or plan to take any other medicines, including medicines obtained without a prescription. This is because Sildenafil can affect the way some medicines work, and some medicines can have an effect on Sildenafil. In the event of a medical emergency, you should tell your doctor, pharmacist, or nurse that you have taken Sildenafil tablets and when you did. Do not take Sildenafil with other medicines unless your doctor tells you that you can.

You should not take Sildenafil if you are taking medicines called Nitrates such as Glyceryl Trinitrate (GTN), as the combination of these medicines may potentially lead to a dangerous fall in your blood pressure. Always tell your prescriber if you are taking any of these medicines that are often used for the relief of angina. You should also avoid taking Sildenafil if you are using Nitric Oxide Donors such as Amyl Nitrite (Poppers).

Patients that take Alpha-blockers (Doxazosin, Indoramin, Terazosin or Prazosin) for the treatment of high blood pressure or prostate enlargement may experience dizziness or light-headedness caused by a sudden drop in blood pressure. This is most likely to happen within 4 hours of taking Sildenafil. In order to reduce the chance of these symptoms occurring, patients should be on a stabilised, regular daily dose of their Alpha-blocker before taking Sildenafil. A lower starting dose (25mg) of Sildenafil may initially be prescribed.

If you are taking a Protease Inhibitor, such as for the treatment of HIV, your doctor may start you on the lowest dose (25mg) of Sildenafil.

You should not take Sildenafil if you are taking a medicine called Riociguat used to treat Pulmonary Arterial Hypertension (PAH).

Do not take Sildenafil:

  • If you are allergic (hypersensitive) to Sildenafil or any of the other ingredients of Sildenafil (listed in the Patient Information Leaflet supplied with your medication)
  • If you are taking medicines called Nitrates (for angina/chest pain) or Nitric Oxide Donors such as Amyl Nitrite (‘Poppers’), as the combination may lead to a dangerous fall in your blood pressure
  • If you are taking a medicine called Riociguat used to treat Pulmonary Arterial Hypertension (PAH)
  • If you have a severe heart or liver problem
  • If you have recently had a stroke or a heart attack, or if you have low blood pressure
  • If you have certain rare inherited eye diseases (such as retinitis pigmentosa)
  • If you have ever had loss of vision due to Non-arteritic Anterior Ischaemic Optic Neuropathy (NAION)

 

Warnings & Precautions

Talk to your doctor, pharmacist, or nurse before taking Sildenafil:

  • If you have sickle cell anaemia (an abnormality of red blood cells), leukaemia (cancer of blood cells), multiple myeloma (cancer of bone marrow)
  • If you have a deformity of your penis or Peyronie’s Disease
  • If you have problems with your heart. Your doctor should carefully check whether your heart can take the additional strain of having sex
  • If you currently have a stomach ulcer, or bleeding problems (such as haemophilia)
  • If you experience sudden decrease or loss of vision, stop taking Sildenafil and contact your doctor immediately

 

You should not use Sildenafil:

  • With any other oral or local treatments for ED
  • With treatments for Pulmonary Arterial Hypertension (PAH) containing Sildenafil or any other PDE5 inhibitors
  • If you do not have Erectile Dysfunction
  • If you are a woman or a male under the age of 18

 

You should tell your doctor if you have kidney or liver problems. Your prescriber may decide a lower dose of Sildenafil is more suitable.

 

Driving and using machinery

Sildenafil can cause dizziness and can affect vision; you should not drive or operate machinery if affected.
