What is Vagifem and how does it work?

Vaginal Dryness is a common condition that is characterised by irritation, discomfort, or itching inside and on the surrounding skin of the vagina as a result of reduced vaginal moisture and lubrication.

In most women, the skin and tissues surrounding the vagina are kept moist and supple by fluids and mucus that are produced by glands at the neck of the womb. This mucus plays an essential role in lubricating and hydrating the vaginal area. The female hormone Oestrogen is responsible for maintaining healthy levels of these fluids, keeping the vagina healthy, elastic, and clean. After the menopause, the ovaries produce less Oestrogen, this natural decline in levels leads to thinning of the tissues around the vagina and a reduction in mucus production, resulting in Vaginal Dryness and its associated symptoms.

One of the most effective treatments for Vaginal Dryness caused by Oestrogen imbalance is the use of Oestrogen-based topical therapies, such as Vagifem. With topical therapy, treatment is applied directly to the affected area so that Oestrogen is restored to the vagina and surrounding tissues. This means that the level of Oestrogen in the bloodstream doesn’t peak in the same manner as with tablets or patches, reducing the risk of side effects.

Vagifem contains an Oestrogen known as Oestradiol. Vagifem is used to relieve menopausal symptoms in the vagina such as dryness or irritation by replacing the Oestrogen which is normally produced in the ovaries of women. Vagifem pessaries are inserted into the vagina, so the hormone is released where it is most needed to relieve discomfort.

Most women will experience relief from their symptoms after about three weeks of treatment. Maximal benefit usually occurs within 1-3 months but can take up to a year. If symptoms have not improved with an Oestrogen-based treatment, then it is important to consider other potential underlying causes.

Oestrogen-based treatments are not always suitable, particularly for those with certain types of cancer or those who are pregnant. In these cases, you may wish to try vaginal moisturisers as an alternative.

How should I use Vagifem?

Before using Vagifem, you should read the manufacturer’s printed Patient Information Leaflet (PIL) from inside the pack. It will give you more information about Vagifem and provide you with a full list of the side effects which you may experience from using it.

Vagifem vaginal tablets are administered intravaginally by use of an applicator. Please see the Patient Information Leaflet (PIL) packaged with your treatment for detailed instructions of how to use Vagifem.

The usual initial dose is one vaginal tablet inserted daily for the first two weeks. After that, the dosage is typically reduced to one vaginal tablet twice a week, with a 3-4-day gap between doses. Treatment can be started on any convenient day.

Along with their useful effects, most medicines can cause unwanted side effects although not everyone experiences them. The unwanted effects often improve as your body adjusts to the new medicine, but please speak with your doctor or pharmacist if they continue or become troublesome. You will find a full list of possible side effects in the Patient Information Leaflet (PIL) supplied with your medicine.

Common side effects (may affect up to 1 in 10 people)

  • Headache
  • Stomach pain
  • Vaginal bleeding, discharge, or discomfort

Please tell the prescriber if you are taking, have recently taken, or might take any other medicines, including medicines obtained without a prescription. This is because Vagifem can affect the way some medicines work, and some medicines can have an effect on Vagifem. Vagifem is used locally in the vagina and is not likely to interact with other medicines although it may interfere with other vaginally applied treatments.

Do not use Vagifem:

  • If you are allergic (hypersensitive) to Oestradiol, any other Oestrogen, or any of the ingredients listed in the Patient Information Leaflet supplied with your medication
  • If you have, have ever had, or are suspected of having breast cancer or cancer which is sensitive to Oestrogens such as endometrial, ovarian, or cervical cancer
  • If you have or have had any unexplained vaginal bleeding in the past 12 months
  • If you have excessive thickening of the womb lining (endometrial hyperplasia) that is not being treated
  • If you have or have ever had a blood clot in a vein (thrombosis), such as in the legs (DVT) or the lungs (pulmonary embolism)
  • If you have a blood clotting disorder (such as protein C, protein S or antithrombin deficiency)
  • If you have or have recently had a disease caused by blood clots in the arteries, such as a heart attack, stroke, or angina
  • If you have or have ever had a liver disease and your liver function tests have not returned to normal
  • If you have a rare hereditary blood disorder known as porphyria

 

Warnings & Precautions

You should tell the prescriber if you have, or have ever had, any of the following problems before you start using Vagifem, as these may return or become worse during treatment. If so, you should consult your doctor.

  • Asthma
  • Epilepsy
  • Diabetes
  • Gallstones
  • High blood pressure
  • Migraines or severe headaches
  • A liver disorder
  • Growth of womb lining outside your womb (endometriosis) or a history of excessive growth of the womb lining (endometrial hyperplasia)
  • A disease affecting the eardrum and hearing (otosclerosis)
  • A disease of the immune system that affects many organs of the body (Systemic Lupus Erythematosus – SLE)
  • Fibroids inside your womb
  • A very high level of fat in your blood (triglycerides)
  • Fluid retention due to cardiac or kidney problems
  • Hereditary and acquired angioedema
  • An increased risk of developing blood clots
  • An increased risk of getting an Oestrogen-sensitive cancer (such as having a mother, sister, or grandmother who has had breast cancer)

 

Stop using Vagifem and seek immediate medical attention if you notice or develop any of the following:

  • Migraine-like headaches which happen for the first time
  • Yellowing of your skin or the whites of your eyes (Jaundice). These may be signs of a liver disease
  • Swollen face, tongue and/or throat, and/or difficulty swallowing or hives, together with difficulty breathing which are suggestive of angioedema
  • A large rise in your blood pressure (symptoms may be headache, tiredness, dizziness)
  • If you notice signs of a blood clot, such as: painful swelling and redness of the legs, sudden chest pain, or difficulty in breathing
  • Any of the conditions mentioned in the ‘Do not use Vagifem’ section above
  • If you become pregnant

 

Pregnancy, Breastfeeding, & Fertility

Vagifem is for use in postmenopausal women only. If you become pregnant, stop using Vagifem and contact your doctor.

 

Driving and using machinery

Vagifem has no known influence on your ability to drive or operate hazardous machinery.
