What is Zyban and how does it work?

Zyban contains the active ingredient Bupropion, a medication that acts as an aid to smoking cessation in combination with motivational support.

Stopping smoking is often associated with intense withdrawal symptoms, including cravings, anxiety, restlessness, headaches, irritability, increased hunger, and difficulty concentrating. Zyban works by altering the levels of certain neurotransmitters in the brain, helping to ease these withdrawal symptoms and increasing the likelihood of quitting. Studies indicate that Zyban roughly doubles the chances of stopping smoking successfully.

Please note that Zyban does not ‘make’ you stop smoking; it is an aid. You still need determination and commitment to succeed and to break the smoking habit.

How should I take Zyban?

Zyban is available from PrescriptionPad as 150mg prolonged release tablets.

Before taking Zyban, you should read the manufacturer’s printed Patient Information Leaflet (PIL) from inside the pack. It will give you more information about the tablets and provide you with a full list of the side effects which you may experience from taking them.

The initial dose of Zyban is one tablet (150mg) taken once daily for the first six days. Starting on day seven, increase to one tablet (150mg) twice daily, unless directed otherwise. Ensure there is at least an eight-hour gap between doses, ideally taking one tablet in the morning and another in the early evening. Do not take more than one tablet at any one time, and not more than two tablets in a day. If you are elderly or if you have certain liver or kidney diseases, your dose of Zyban may be different – your prescriber will advise.

You must set a target date to stop smoking within the first two weeks of treatment with Zyban, preferably in the second week. This allows Zyban to build up in your body before you stop completely.

A full course of treatment lasts for 7-9 weeks in total. If at seven weeks no effect is seen, treatment should be discontinued.

Zyban tablets should be swallowed whole. The tablets should not be cut, crushed or chewed as this may lead to an increased risk of adverse effects including seizures. It is not important whether you take Zyban before or after food, but it is best if you try to take your doses at the same time each day. This will help you to remember to take the tablets regularly.

Insomnia is a very common adverse event which can be reduced by avoiding bedtime doses of Zyban (provided there is at least 8 hours between doses).

If you forget to take a dose, leave out the missed dose but make sure that you remember to take your next dose when it is due. Do not take two doses together to make up for the forgotten dose.

If you accidentally take more Zyban than prescribed, you may experience an increase in side effects and their severity. Contact your doctor if you take more tablets than you should.

After your course of treatment, your prescriber may suggest that you gradually reduce the dose over a few days before you finally stop taking the tablets.

Along with their useful effects, most medicines can cause unwanted side effects although not everyone experiences them. The unwanted effects often improve as your body adjusts to the new medicine, but please speak with your doctor or pharmacist if they continue or become troublesome.

Below is a list of the most common and the most serious side effects associated with Zyban. You will find a full list of possible side effects in the Patient Information Leaflet (PIL) supplied with your medicine.

 

Stop taking Zyban and see a doctor immediately if:

* You have fits/seizures. Approximately 1 in every 1,000 people taking Zyban is at risk of having a fit. Symptoms of a fit include convulsions and usually loss of consciousness. Someone who has had a fit may be confused afterwards and may not remember what has happened. Fits are more likely if you take too much, if you take some other medicines, or if you are at higher than usual risk of fits

* You have an allergic reaction. Rarely (up to 1 in 1,000) people may have potentially serious allergic reactions to Zyban. Signs of allergic reactions include:

   – Skin rash (including itchy, bumpy rash). Some skin rashes may need hospital treatment, especially if you also have a sore mouth or sore eyes Unusual wheezing or difficulty in breathing

   – Swollen eyelids, lips, or tongue

   – Pains in muscles or joints

   – Collapse or blackout

* You have lupus skin rash or worsening of lupus symptoms. Lupus is an immune system disorder affecting the skin and other organs

* You have Acute Generalised Exanthematous Pustulosis (AGEP). Symptoms of AGEP include rash with pus filled pimples/blisters

 

Very common side effects (may affect more than 1 in 10 people)

  • Difficulty sleeping (make sure you don’t take Zyban near to bedtime)

 

Common side effects (may affect up to 1 in 10 people)

  • Feeling depressed
  • Feeling anxious or agitated
  • Difficulty concentrating
  • Feeling shaky (tremor)
  • Headache
  • Nausea
  • Vomiting
  • Stomach pain or other upsets (such as constipation), changes in the taste of food, dry mouth
  • Fever, dizziness, sweating, skin rash (sometimes due to an allergic reaction), itching

 

Effects of giving up smoking

People who stop smoking are often affected by nicotine withdrawal. This can also affect people taking Zyban. Signs of nicotine withdrawal include: difficulty sleeping, tremor or sweating, feeling anxious, agitated or depressed, sometimes with thoughts of suicide. You should talk to your doctor if you have any concerns about how you feel.

Please tell the prescriber if you are taking, have recently taken or might take any other medicines, including medicines obtained without a prescription. This is because Zyban can affect the way some medicines work, and some medicines can have an effect on Zyban. In the event of a medical emergency, you should tell your doctor, pharmacist, or nurse that you have taken Zyban tablets and when you did. Do not take Zyban tablets with other medicines unless your doctor tells you that you can.

There may be an increased risk of seizures if you are taking any of the following medications. If you are using any medicines on this list, consult your doctor before starting Zyban:

  • Medicines for depression or other mental health problems
  • Theophylline for asthma or lung disease
  • Tramadol, a strong painkiller
  • Antimalarial medication
  • Stimulants or other medicines to control your weight or appetite
  • Steroids (except creams and lotions for eye and skin conditions)
  • Antibiotics called quinolones
  • Some types of antihistamines
  • Medicines for diabetes

 

Tell your doctor or pharmacist if you are taking any of the following medicines:

  • Medicines for depression or other mental health problems
  • Medicines for Parkinson’s disease (such as Levodopa, Amantadine, or Orphenadrine)
  • Epilepsy/seizure control medication (Carbamazepine, Phenytoin, Valproate…)
  • Some medicines used to treat cancer (such as Cyclophosphamide, Ifosfamide)
  • Ticlopidine or Clopidogrel, mainly used to treat heart disease or stroke
  • Beta-blockers (such as Metoprolol)
  • Medicines for irregular heart rhythm (such as Propafenone, Flecainide)
  • Ritonavir or Efavirenz, for treatment of HIV infection
  • Tamoxifen, used to treat breast cancer
  • Digoxin, for your heart
  • Monoamine Oxidase Inhibitors (MAOIs) – medicines used to treat depression or Parkinson’s disease, such as Phenelzine, Selegiline, or Rasagiline

 

The dose of certain medications may need to be adjusted when you stop smoking. The chemicals absorbed through smoking can reduce the effectiveness of some medicines, so when you quit, your dosage might need to be lowered to avoid potential side effects. If you’re taking any other medications, consult your doctor if you experience any new symptoms that could be side effects.

Do not take Zyban:

  • If you are allergic (hypersensitive) to Bupropion or any of the other ingredients of Zyban (listed in the Patient Information Leaflet supplied with your medication)
  • If you are taking any other medicines which contain Bupropion
  • If you have a condition that causes fits (seizures), such as epilepsy, or if you have a history of fits
  • If you have an eating disorder or had one in the past (for example, bulimia or anorexia nervosa)
  • If you have severe liver problems, such as cirrhosis
  • If you have a brain tumour
  • If you are usually a heavy drinker and you have just stopped drinking alcohol, or are going to stop while you’re taking Zyban
  • If you have recently stopped taking sedatives or medicines to treat anxiety (especially Benzodiazepines or similar medicines), or if you are going to stop them while you’re taking Zyban
  • If you have a bipolar disorder (extreme mood swings) as Zyban could bring on an episode of this illness
  • If you are taking medicines for depression or Parkinson’s disease called Monoamine Oxidase Inhibitors (MAOIs), or have taken them in the last 14 days

 

Warnings & Precautions

Talk to your doctor, pharmacist, or nurse before taking Zyban if you have a condition called Brugada syndrome (a rare hereditary syndrome that affects the heart rhythm) or if cardiac arrest or sudden death occurred in your family.

Fits (seizures) – Zyban has been shown to cause fits in about 1 in 1,000 people. Fits are more likely: if you regularly drink a lot of alcohol, if you have diabetes for which you use Insulin or tablets, if you have had a serious head injury or a history of head trauma. If any of these applies to you, don’t take Zyban unless you have agreed with your doctor that there is a strong reason for doing so. If you have a fit during treatment: stop taking Zyban and don’t take any more, see your doctor.

You may have a higher risk of side effects if you have kidney or liver problems, or if you are aged over 65. You will need to take a lower dose and be checked closely while you are taking Zyban

Some people taking Zyban have experienced hallucinations or delusions (seeing, hearing, or believing things that are not there), disordered thoughts, or extreme mood swings. These effects are more likely in people who have had mental health problems before.

Some people become depressed when they try to stop smoking; very occasionally, they may think about committing suicide, or try to do so. These symptoms have affected people taking Zyban, most often in the first few weeks of treatment. If you feel depressed or think about suicide: contact your doctor or go to a hospital straight away.

The use of medicines for depression together with Zyban can lead to serotonin syndrome, a potentially life-threatening condition.

Some people taking Zyban have developed high blood pressure which needs treatment. If you already have high blood pressure, it can become worse. This is more likely if you are also using nicotine patches to help you stop smoking. You should check your blood pressure before you take Zyban and while you are taking it, especially if you already have high blood pressure. If you are also using nicotine patches, your blood pressure needs to be checked every week. If your blood pressure increases, you may need to stop taking Zyban.

Some people find they are more sensitive to alcohol while taking Zyban. Your doctor may suggest you do not drink alcohol while you’re taking Zyban, or try to drink as little as possible. If you do drink a lot now, don’t just stop suddenly, because that may put you at risk of having a fit.

Zyban may interfere with some urine tests to detect other drugs. If you require a urine test, tell your doctor or hospital that you are taking Zyban.

 

Pregnancy, Breastfeeding, & Fertility

Zyban should be avoided during pregnancy and when breastfeeding.

 

Driving and using machinery

Some of the side effects of Zyban, such as feeling dizzy or light-headed, may affect your concentration and judgement. If you are affected, don’t drive or operate machinery.
